Hi,
Can someone upload your intake paperwork? Waivers, contraindications, etc. And everything in the back of the User Manual? The google docs won't allow copy/paste and it's quite a bit to type out, and I figured one of you would have already done the legwork. Thanks!

We don't have "intake" paperwork per-se
We do have a personal health maintenance portal in development which we're calling mycare or prime.liveo2.com/mycare
